The Heart and Science of Cognitive Wellness

Senior Cognitive Wellness was created to provide expert consultation, cognitive health assessments, and compassionate guidance for individuals and their families.

Led by a Board-Certified Gerontological Registered Nurse and Certified Dementia Practitioner with 30 years of experience, we specialize in cognitive and psychosocial assessment, dementia care planning, and ongoing navigation of next steps and resources.

Our goal is to bridge the gap between early concerns and a specialist’s diagnosis by offering timely assessments, clear clinical insight, and supportive recommendations.

Helping families navigate complex care needs. We provide direct support and strategies for those caring for loved ones. We offer tools for managing challenging behaviors and identify local resources to help coordinate care.

Bridging the Gap for your loved ones

We bridge the gap between initial concerns and a specialist's diagnosis by providing cognitive screenings and actionable clinical data. All findings can be shared with your primary care provider to help streamline your next steps.
